How do I list my chronic disease patients?
• If you are already using the EMR CDM encounter flowsheet features, the CDM Patient List report can be generated quickly with the following steps. NOTE: If you are not yet using the EMR CDM encounters, not all the data for the patient lists is available in the system. To find out how to identify CDM patients read this FAQ item “How do I list CDM or complex care patients with unbilled incentive fees”. We recommend that you use the EMR CDM encounters to save time during the encounter and save time when you would like reports on your CDM patient population. In the main screen select Reports, CDM Patient Lists. • Select the Doctor code from the drop-down list. • Select then the condition you want to search for (or All Conditions for a complete list). • Click OK and it will list all patients that have encounters in their chart for the appropriate Chronic Disease.
