How do I know which version of the IRB approved consent/assent form to use?
A. When the IRB approves a consent/assent document, the Office of Research Integrity stamps the document with an “IRB Approval” stamp. The stamp on the first page will also contain the approval period for which the document is valid. Each time you receive a newly approved consent/assent document, all previously approved consent/assent documents are then considered invalid for enrollment of subjects. Note the first date in the approval period in the “IRB Approval” stamp (i.e., In the approval period 5/29/10 – 5/28/11, the “first date” is 5/29/10 ); the consent/assent document with the most recent “first date” is the currently approved version.
