How do I know if my child is eligible for HealthWave?
You or your child may qualify for HealthWave if you are a Kansas resident, you are a U.S. citizen or a documented non-U.S. citizen, and you meet current income guidelines. To find out if you are eligible, complete a HealthWave application, and mail it in. Applications may be downloaded from the HealthWave Web site, or one will be sent to you through the mail by calling 1-800-792-4884, or 1-800-792-4292 (TTY). Applications are also available at any Regional SRS office or community Access Point.
