For an individual to join or sign up for a program a person would need to fill out a program registration form, fill out and sign a waiver form, and send in a $25 deposit. For a team to join a league or tournament a team captain or coach must fill out a program registration form, a roster form, have each participant fill out and sign a waiver, and send in a $100 deposit. All of these forms are available on our website.
