How do I initiate a VSDP short-term disability claim?
Anytime you may be absent for more than 5 work days due to an illness, accident or work-related injury call Unum at 1-800-652-5602. Please be prepared to answer some basic questions for Unum such as your last day worked, physician’s name, physician’s address and physician’s telephone number. Unum will mail an information packet with forms included to your home address.
