• All prime contractors will be required to submit the online FHWA 1589 form by the 10th of each month. In order to submit the FHWA 1589 form each contractor will need to get a user id and password which can be obtained by filling out the Account Request Form at -> http://www.dot.state.oh.us/Divisions/ContractAdmin/Contracts/Pages/AccountRequest.
