You need to return to the last medical facility you were seen at, go to their medical records department, and sign a release of information form to get your medical records.

You simply go to a new doctor that will accept you on his books…..You give him/her your details and information of your previous doctor……….He/She will send off for a search of your documents….This can take some time……A new card will be issued by the doctor with your old national health number issued from the records department……Simple as that……. We have just done this after returning from Spain……..As for the previous answer…..Doctors will not give you your records………Plus…..You are only allowed to see your records going back 3 months….no more……..

with the NHS you just sign in with your local surgery and it is up to them to trace your records – be warned some of them are better than others and it may take some time (mine took five months to catch up with me when I had all of my details and those of the children)

Look when I tell the doctor that my last doctor said so and so he looks at me like I am an idiot, but you can go to one and ask for your records. how true they are is a different matter!
