If your child(ren) need additional immunizations, see your doctor or the Public Health Department gives free immunizations. HomeSource will need to have the immunizations recorded on the Oregon Certificate Of Immunization Status. This form is available at HomeSource. Each time your child has additional immunizations, please update the form on file at HomeSource.
