Pick up the form in Room 120A. You will need to get a course description of the course from the accredited university or college. You will then need to make an appointment with your Departmental Undergraduate Director to get approval. This must be done prior to taking the course. The form and the course description will be kept in Room 120A until the office receives an official transcript from you (in a sealed envelope). After receiving the transcript it will be entered into ISIS. A confirmation letter will be placed in your departmental mailbox.
