How do I get a Glucometer for checking my blood sugar at home?
You will need 2 prescriptions from your Primary Care Provider (either handwritten or entered into the computer). One will be for your supplies and you will pick them up at the pharmacy. Bring the second prescription (for the monitor/machine) to the DMC with your supplies. You will be provided with a monitor and shown how to use it.
