Look on the back of your lock to see if it has a serial number. Without a serial number, Master Lock cannot provide a combination. Go to masterlock.com and search for “Lost Combination Form.” Print out the form. Fill it out and have it notarized. Inmates can have a prison official sign it instead of a notary public; however, the form must be printed on official prison letterhead. Photocopy the back of the lock, ensuring that the serial numbers are visible. In addition, write the serial number again in pen or marker somewhere on the photocopy, without obscuring the lock image.
