How do I fill out a Supply Chain Management Service Request Form?
You must sign on to the University of Calgary My UofC portal. The link can be found at the top of the University of Calgary homepage. Go to the area titled “Enterprise Systems”, which leads you to another title called “Supply Chain Management” and finally to a title called “Supply Chain Management Service Request Forms”. Once the authorizer you have noted at the bottom of the form has approved the submission, the request will be completed. We cannot pick up your request until this request is approved! If you or your approver are not listed on the teleweb, please contact the teleweb administrator for your department to have them added.
