You can download a copy of the Commission’s complaint form from this website. In the complaint, you must (1) allege that the person named violated Chapter 104, Chapter 106, or Section 105.071, Florida Statutes, (2) state specific facts to show that the person named violated an election law, (3) state facts that occurred no more than two years before the date you file the complaint, and (4) sign the complaint under oath in the presence of a notary public. When completed, mail the complaint with your original signature to the Florida Elections Commission at 107 West Gaines Street, Suite 224, Collins Building, Tallahassee, FL 32399-1050. See Rule 2B-1.0025(4), Florida Administrative Code. Because the signature is not original, the Commission will not accept faxed complaints.
