• Eligible expenses incurred by multiple eligible dependents with the same expense type, date of service and final patient responsibility should all be filed as individual line items in one express claim or all on the same standard claim form. Be sure to include your EOB or itemized statement for each expense which shows the claimed items are for multiple services and dependents. If claims are filed separately for each dependent they could be denied as “possible duplicates”. i.e. My dependents, Joe & Susie both visit the dentist on 3/23/2010. After insurance, my financial responsibility is $30.00 for each visit. I will need to file these two expense reimbursements at the same time and on the same form.
