• By telephone – call the Aetna LTD Claim Office at 877-352-3862, Monday – Friday, 8:00 a.m. – 7:00 p.m. Central Time • Online – submit your claim at www.wkabsystem.com • By completing a Claim Form – obtain a form from Forms Download or by using our contact form. You must give Aetna Life Insurance Company (Aetna) written Statement of Claim within 30 days after the first day you miss work due to a disability. When Aetna receives your Statement of Claim, you will be sent forms for providing Proof of Loss within 15 days. To obtain a Claim Form either: • Download a form from our Forms Download page. • Request one using our contact form. Important Note: If you incurred a claim prior to April 1, 2007, The Hartford will service your claim.
