How do I establish my need for Future Drug Treatment or Methadone maintenance for continued dependency on OxyContin or Oxycodone?
First, you must have registered for the Future Drug Treatment Benefit by June 15, 2007, with the Claims Administrator. Second, you must have been diagnosed by a qualified professional as needing continued drug treatment for substance abuse or addiction resulting from the ingestion of OxyContin or oxycodone. To claim this benefit, you must submit a completed Diagnosis of OxyContin or Oxycodone Chemical Dependency form to be considered for future drug treatment and/or Methadone maintenance.
