Please complete and submit the Field Experience Request Form, located under “Hosting an MPH Student” on the MPH Program website homepage. The form can be submitted electronically to Dr. Michael Cates, MPH Program Director, at cates@k-state.edu, or mailed to the MPH Program Office, 1 Trotter Hall, Kansas State University, Manhattan, KS 66506.
