You can ensure you are included in the publication by: (i) filling out and returning your Data Verification Form (either by mail or online); or (ii) phoning Harris Connect directly in response to a postcard request sent by mail or e-mail. Even if you have not received a postcard, you can call Harris Connect at 1-888-869-0786. In order to be included, you must respond no later than September 8, 2008.
