In order to enroll a patient, simply fill out the BioScrip enrollment form and fax it to BioScrip at 800-210-6256. You can also call BioScrip at 800-724-6996 to obtain the enrollment form. BioScrip will follow up and arrange for delivery of the medication to your office.
