Once your employer enrolls your company in the program, information will be made available to you concerning the enrollment period and who is eligible to enroll. You will be given the website address to apply for your benefit. Once you complete the Enrollment Application your employer will be asked to approve your eligibility. After that approval your benefit will be processed.
