How do I Enroll in Fidelis Care at Home?
Participation in Fidelis Care at Home is voluntary. For information on how to enroll in Fidelis Care at Home, call 1-800-688-7422. A Nurse Care Manager will visit the potential Fidelis Care at Home applicant to discuss Fidelis Care at Home’s services and the enrollment process, and to gather information about and assess the healthcare needs of the individual. When the member is enrolled, the Nurse Care Manager assigned to them will create a personalized plan of care by coordinating with the physician, the member, and their family members and/or other caregivers to determine how to best meet the member’s healthcare needs. Fidelis Care at Home membership will normally begin on the first of the month following the Plan’s submission of its assessment and other necessary documents to the Local Department of Social Services by the 20th day of the prior month.
