To enrol in the Optional Life Insurance plan, complete an application form. Once your eligibility has been established, you must complete a Sun Life Statement of Health form and submit it directly to Sun Life. Under the Optional Life Insurance plan, dependent children are automatically covered for $5,000 for every $25,000 of spousal coverage, or, if you are a single parent, $5,000 for every $25,000 of employee coverage.
