Once an entry is submitted using the NPI reporting crosswalk tool, all changes require HP Enterprise Services intervention. HP Enterprise Services Provider Enrollment can be reached in-state toll-free at (800) 457-4454 or local and out-of-state at (501) 376-2211. Select option 0 for “Other inquiries” and then option 3 for “Provider Enrollment” when prompted.
