Criminal History Records information is only available from the Illinois State Police. Information is located at their web page: http://www.isp.state.il.us/crimhistory/crimhistoryhome.cfm. You also may obtain a Uniform Conviction Information Act Information (UCIA) Sheet by ordering the UCIA request forms via the internet at http://www.isp.state.il.us/services/uciaformreq.cfm.
