How do I complete a disclosure application form in conjunction with a Criminal Records Check?
We will send you a disclosure application form populated with the information you have provided for you to check and sign. You then take it to your local Post Office, together with the Veri-fy form and the documents you have selected to confirm your identity and current address. Their staff will check your documents, complete the Veri-fy form and post the disclosure application form and Veri-fy form to us for processing.
