How do I complain about the quality of care provided by a doctor/facility in the PHCS provider Network?
We take the quality of our Network very seriously at PHCS and have created mechanisms to investigate, resolve and track any complaints about the quality of providers that participate in our Network. You may file a grievance regarding the quality of care received from a PHCS provider by contacting our Corporate Quality Management staff. Please send your letter of grievance as well as a completed copy of the attached form to: Private Healthcare Systems, Corporate Quality Management Department, 1100 Winter Street, Waltham, MA 02451.
