How do I change the bank account or Pre-Authorized Checking (PAC) information on my policy?
[top] To change the bank account on your bank draft payment plan, mail us a completed bank authorization form along with a voided check from the bank account that will be drafted. Please note, if premiums are due, a check for the premium should be sent with the authorization form. To download this form, you will need a PDF reader.
