How do I change my e-mail address, password, address, phone number, or account preferences?
Follow these steps: • Log in to myProvidence with your e-mail address and password. • Click on the “My Account” link at top. • On the “Update personal information” link and submit the changes to your account. Please note: myProvidence uses the e-mail address you provide for critical communications related to your account, including notifications when you have unread mail in your message center inbox. *Notes to Providence Health Plan members: • Changes to your myProvidence account will not be reflected in your personal information on file with Providence Health Plan (PHP). If you have employer group benefits please update your information with your benefits administrator. • If you are an Individual and Family Plan member, contact Customer Service to update your information. • In order for you to access myProvidence health plans services the following information must match what is shown on your Providence Health Plans member ID card: your name, birth date, member number and group number
