Amendment to a Birth CertificateChanges such as correction of spelling errors, adding information to previously left blank items, or correcting the spelling of parental names or their statistical information are all errors that can be amended on a birth certificate. To make any of these amendments, complete an Affidavit to Amend a Record (Form VS 24) and add any additional documentation, specific for each case.Changes to a Birth CertificateA court order (decision or adjudication) must be obtained to make any changes to the name of the registrant on the birth certificate or to make any changes to the parents’ names on a child’s birth certificate. To make the change, complete an Application for Amendment of Birth Record to Reflect Court Order Change of Name (Form VS 23) or an Application to Amend a Birth Record—Adjudication of Facts of Parentage (Form VS 21); add a certified copy of the court order and a copy of the current birth certificate.
