How do I cancel a Proactive Disclosure Service Prototype (PDS) enrollment?
Log in to the Integrated Querying and Reporting Service (IQRS) located in the Data Banks home page, and click Manage Subjects on the Proactive Disclosure Service screen. Follow the on-screen instructions to cancel an enrollment. Complete the Submitter Certification screen, and click Submit to Data Bank(s). Enrollment confirmations for canceled enrollments are available for four years from the date of cancellation.
