How do I become a patient at the Robert Mapplethorpe Residential Treatment Facility?
Since the RMRTF is a Nursing Home, you have to be referred by a medical provider. You will need the following for admission: 1. A New York State Patient Review Instrument Screen completed by a Registered Nurse. It indicates whether this setting would be appropriate for your care needs. 2. Active Medicaid (not affiliated with any managed care program). 3. A recent PPD/Chest X-ray ruling out TB. 4. Medical information including complete diagnosis history, HIV status, lab reports, radiology reports (if applicable), recent psychiatric information (if applicable), substance abuse history and, if on methadone maintenance, where you are receiving treatment. 5. Financial information including SSI or SSD payments. 6. Psychosocial information.
