The first step is to complete an application form, available on-line. If you prefer, you can request an application by mail or download one from our web site or call us at (888) 747-7600. You must also complete our testing process, but you can either come into the office or finish most of it on-line at your convenience. We’ll also need copies of key documents, such as your license, CPR, TB skin test and health statement.
