To be considered for participation in our networks, you will need: 1. An assigned Blue Cross and Blue Shield of Florida (BCBSF) provider number. If you do not have one, please call (800) 727-2227 and say “More Choices”, then “Provider Number” to request an application or you may also complete the Provider Registration Form. If you already have a number, proceed to step two (2). 2. Send us a letter indicating you are interested in participating in our network.
