Read and complete the application which is located on the bottom of the page . Be sure to attach a typed biographical statement, goals statement and a statement on your financial obligations. All students must separately apply to ASU, College of Nursing and Health Innovation for admission into the Upper Division Nursing Program (BSN), RN-BSN program or DNP and meet the set criteria. REQUIRED: Certificate of Indian Blood (CIB). Awarding of the LEVDA HUITT CARPENTER Scholarship is strictly based on the decision of the ASUN Advisory Committee recommendation to the Huitt Foundation, the applicant’s admission status into the appropriate Nursing Program, and available scholarship funds.
