How do I apply for Optional Practical Training (OPT)?
The OPT application is essentially a two-step process. Step 1: Submit to the Directorate of International Student Advising the following documents: • Copy of the last I-20 issued to you. • Completed DISA Employment Information Form • letter on University letterhead from the academic adviser indicating the current status (i.e., whether student has completed all requirements for the degree but thesis) and the expected date of completion of studies Step 2: Pick up the new I-20 with the DISA recommendation for OPT. Submit within 30 days the following documents to the U.S. Citizenship and Immigration Services office. • Check for $340 made payable to the U.S. Department of Homeland Security • Completed I-765 (original) or from DISA • ER-750 – Statement stating that all copies are true copies of the original documents with your signature and date.
