How do I apply for HealthWave?
Applications can be downloaded and printed from your computer by clicking here, or you may call the HealthWave Clearinghouse at 1-800-792-4884 and ask to have an application mailed to you. There is also an on-line application you may access at by clicking on the “Apply for Services ” button on the left side of the screen. Application forms are available at any Regional SRS office and many SRS Access Points in the community.
