How do I apply for a Permanent Nantucket Disabled Parking Permit?
Please call the Nantucket Commission on Disability for an application and one will be sent to you in the mail, or you can receive an application at your local Nantucket physician’s office. You will complete the front side of the application and have your physician complete the reverse side.
