This is explain in the Calendar here. The Registrar’s Office has information here, with a link to the Carleton medical form which you may want to give to your doctor.

If you miss your final exam for medical reasons, submit a Deferral of Examination or Assignment Application form to the registrars office. Section 2.5 of the calendar discusses the required medical documentation.
