How do I add a new field for the displayed changed-password entry?

Once the user changes its password a new page will be displayed containing the new password. To add a new field, let’s say “Phone number”, just enter the new the code $entry[$PHONE] in the appropriate order in two places: near line 1162 and near line 1168.
