How do I access necessary forms to complete the application process?
All forms can be downloaded from the Laboratory Schools website. You want the link titled “Process and Forms” from the Admissions section. In an effort be environmentally conscious, we are no longer printing application materials. Instead, all necessary forms are available for download from our website HYPERLINK “http://www.ucls.uchicago.edu” www.ucls.uchicago.edu. If you do not already have the software to access pdfs, you may download the forms at no cost from the Adobe Acrobat Reader page.
