HOW DO EMPLOYEES DROP DEPENDENTS?
Employees wishing to drop dependents from coverage should complete section 6 on the POLICYOWNERS REQUEST FOR POLICY CHANGE form MP-2 (6/81) indicating Drop Dependent Coverage and specifying which dependents if not all that should be dropped. Upon receipt of the request our records will be updated and a new payroll authorization reflecting the premium change will be forwarded for the employee to sign. The request for canceling dependents should be received in our office 30 days prior to the desired cancellation date.
