How Do Care Management Processes Help?
Care management processes are a part of the movement toward integrated, team-based care. “The main point of our article is not that there are incompetent doctors, but that they are practicing in systems of care that are outdated,” says Stephen Shortell, professor and dean of University of California-Berkeley’s School of Public Health and principal investigator of the study. “If I’m getting my diabetes treated by a doctor who has a disease registry, the doctor can remind me when I need to come in, rather than me trying to remember when to make the appointment or to keep track of a slip of paper the office hands to me,” explains Shortell. “If I have a lot of problems and I’m in a patient self-management education program, they’ve trained me how to monitor my insulin carefully and who to call if I have trouble, and I feel more secure about taking care of myself.” Lawrence Casalino, a physician and assistant professor of health studies at the University of Chicago and lead author of the pa
