How do arrange AdvantageCare Home Health Inc. for myself or my family member?
● Self referrals are welcomed ● Referrals are accepted from friends, family, social workers, or hospital discharge planners by simply calling our office ● All home care services must be approved by your physician ● After approval is received a registered nurse will conduct an evaluation visit to your home to determine eligibility and services needed ● When eligibility and need is determined, an individualized plan of care is developed with the client and the client’s physician
