How did the survey format get decided, including racial and ethnic categories, etc.?
We have largely based our diversity survey on the format used by the official United States Census. There is some debate about how surveys should address these issues. By following the U.S. Census format, SEIU Local 925 does not necessarily endorse that approach. It is simply the standard which most employers and government agencies use.
