On the form, record the date an associate takes a holiday whether it is the date designated by the Governor or not. A quick glance at the form will reveal to the viewer which holidays have not yet been taken. The form will help prevent the same holiday being unknowingly taken twice by an associate. Remember: Holidays cannot be taken in advance of date assigned by the Governor.
