Forms mailed to private residences will be in English only. (Exception: in Minnesota, selected blocks in Minneapolis, St. Paul, and Worthington will receive bilingual Spanish/English forms). You can also request and submit an official form in Spanish, Korean, Vietnamese, Simplified Chinese, and Russian.
