Microsoft Word or text version. • Attach an original void cheque (or an original letter from your financial institution) and send to the address noted on the form. • We recommend you register for our e-mail notices by providing your e-mail address on the application form. On the date of each payment you will receive an automatic e-mail providing details of the electronic payment including: amount, date, description, invoice numbers, etc. These are available at no cost as a reconciliation tool.
