How can I receive additional paper assessments or forms?
The number of rater assessments you received was predetermined to ensure you receive the most complete and valuable feedback. However if you still wish to use more raters, request additional forms by accessing your Internet assessment account (details given in your original e-mail). Use the “rater administration” screen to enter the name, type of form (paper or Internet) requested, and address if paper, or e-mail address if Internet, and relationship of the person to whom the form should be sent.
