Download the Authorization to Use and Disclose Protected Health Information form. Have the patient or guardian complete the form and submitted it to Patient Financial Services along with a copy of a valid photo I.D. In Section 3 of the form, indicated the name of the Representative that will be receiving the copy of the billing statement.
