How can I order inspection certificates and forms MV-50?
On-line (inspection certificates only) Inspection stations can order inspection certificates on-line. You must provide your facility number, the type of stickers you that you need, and an e-mail address to receive your confirmation. Use a credit card to pay for the order. The order normally arrives in two weeks if you use standard postal delivery, or less than two weeks if you use an express mail service. There is a fee of $10 for express mail service. By mail Download form VS-113I to order inspection stickers, or download form VS-114I to order a form MV-50 book. You must send a completed form MV-50 book when you order a new form MV-50 book. If you use an express mail service to send your order, make sure that you use the correct address. Include an additional $10.00 in your payment to receive your order by an express mail service. Make sure that you use the correct mailing address for your order.
